The Ratings Settings: You can add more evaluation levels to the assessment by selecting/deselecting the checkboxes here. Note that the top and bottom ratings (“Perfect” and “Unsatisfactory”) are always enabled and cannot be disabled.
The Reasons Settings: This list controls the reasons the evaluator can cite for giving the trainee a rating lower than Perfect on a particular step in the procedure. “Skipped,” “Not to Standard,” and “Completed Out of Order” are selected by default. These can be removed from the list by deselecting the checkboxes next to them.
Custom Reasons: You can add more reasons to the list by entering them in the Plus line here. They can be removed by clicking the Subtract button next to the entry.
